Models
1) Card for People with Early Stage Dementia
This card is particularly useful for people who are in the early stages of dementia, and might need help and understanding at certain times, eg to show a bus driver who may be getting impatient.
Based on a Help Card developed by Alzheimer's Scotland - Action on Dementia.

2) Personal Identity Card
If a person with dementia is unable to use the Card for People with Early Stage Dementia, the Identification Card could be used. A family member or a carer should fill in the details on this card and ensure that the person with dementia carries it.

3) Companion Card
Available to anyone who is a carer of someone with dementia. The Companion Card is helpful if a situation arises in public where the behaviour of the person with dementia may not be understood. As it may not always be possible or appropriate for a carer to provide an explanation of the behaviour, the card can be handed over.
