Short Description
This government scheme (TGA) aims to improve the standard of medical devices available in Australia and to minimise the adverse effects of problems with devices. It provides an additional means by which unsafe products or procedures can be identified quickly so that appropriate action may be taken. It encourages voluntary co-operation between users, government and industry and should be used in conjunction with local reporting channels.

WHAT IS A THERAPEUTIC DEVICE?
Any material, instrument, machine, appliance, implant, or component of these used in the delivery of health care.

WHAT SHOULD BE REPORTED?
Typical problems may include deficiencies in labelling, instructions or
packaging, defective components, performance failures, poor construction or design, (anything that adversely affects the use of a device). Suggestions for rectifying or improving product performance would be appreciated.
Reports are investigated by technical staff in conjunction with the
responsible company. The reporter is informed of the outcome of the
investigation.
Problem Report Forms are available from the Therapeutic Devices Branch of the Therapeutic Goods Administration.
